Congratulations! You’ve taken the first step towards a beautiful and confident smile by making a consultation appointment with Dr. Lee.

Next Step

Now What

Your first appointment with Dr. Lee will last between 30 and 45 minutes. When you arrive at our office, you’ll be greeted by our friendly staff and asked to fill out a patient information/medical and dental history form. Please come five to 10 minutes early so you have time to complete it.

Once you’ve finished the paperwork, Dr. Lee will conduct a thorough clinical examination of extra-oral (facial) and intra-oral (mouth) conditions. Diagnostic x-rays can be taken if needed so Dr. Lee can analyze them.

Dr. Lee will listen to your main concerns and answer any questions you may have. H’ll fill you on his exam findings and discuss how he can improve your teeth alignment, bite and smile esthetics. Our treatment coordinator will discuss cost, insurance coverage, our in-office, no-interest financing and payment options, as well as outline what to expect with your treatment. You’ll leave feeling confident in your choices.

Think of all of Your Questions and Concerns About Your Teeth and Braces Treatment

It’s helpful to make a list of your questions and concerns about your teeth alignment or bite problems and treatments. Dr. Lee welcomes questions and really listens. He will explain all of the treatment options in detail so you can understand what is involved and what you can expect from each one.

Insurance

Call your insurance company prior to your visit and get some details about coverage so we can give you an accurate breakdown of the fees at your consultation. You’ll want to ask your insurance company the following questions:

- Does my plan cover orthodontic treatment?
- What is the percentage of the coverage? What is the limit?
- Do you accept assignment (can the dental office bill the insurance directly)?
- Is there an age limit?

New Patient Forms

If you can, fill out new patient form prior to your visit.
